Sussex Community NHS Foundation Trust’s (SCFT) app – connecting our patients to our services. If you’re downloading this app, you’ve probably heard about it from one of our staff. If not, you’re welcome to browse what’s on here, but more information about what we do can be found on our website. This library contains the apps some of our teams have created for patients. Not all our services have an app yet but as they are developed they will be added to the library. SCFT are the main provider of Community NHS health and care across Brighton & Hove, East Sussex, High Weald Lewes and Havens and West Sussex. Our expert teams provide essential medical, nursing and therapeutic care to adults, children and families. Our aim across all of our services is to give people choice about the care they receive and provide certainty that when they need us, wherever they are, we will meet their needs with services of a high quality that are safe, effective and compassionate, and that are offered with respect.
